Nurse Practitioner – Primary Care (ESKD Focus) – Coastal Southeastern Virginia

 

Opportunity Highlights

  • Lead full-time onsite Nurse Practitioner role supporting an innovative, clinic-based model of care for patients with end-stage kidney disease (ESKD)
  • Practice at multiple dialysis centers within the Chesapeake/Norfolk area, bringing primary care directly to patients where they receive treatment
  • Monday–Friday schedule with no nights, weekends, or on-call duties
  • Flexible “round-and-go” model gives you control over patient visits and clinical priorities
  • Help shape a new care delivery model—serve as the clinical lead and collaborate closely with nephrologists, dialysis staff, and care coordination teams
  • Build meaningful long-term relationships with a stable, consistent patient panel

 

Responsibilities

  • Provide comprehensive primary care services to adult patients with chronic kidney disease and comorbidities
  • Average 10–12 patient visits per day, focusing on prevention, chronic disease management, and coordination of care
  • Conduct annual health evaluations, medication management, and care plan development
  • Collaborate with nephrologists, dialysis teams, and other providers to reduce hospitalizations and improve health outcomes
  • Participate in care redesign feedback to optimize the program’s clinical model

 

Compensation & Benefits

  • Competitive base salary: $130,000 – $170,000
  • Productivity bonus tied to quality outcomes and care metrics (up to 60% of base)

Candidate Requirements

  • Minimum 5 years of experience as a nurse practitioner in primary care, internal medicine, or gerontology
  • Licensed to practice independently in Virginia
  • Active DEA license and experience managing chronic conditions such as diabetes, hypertension, COPD, CHF, and hyperlipidemia
  • Experience in dialysis or nephrology a plus
  • Proven ability to work independently while collaborating effectively with a broader care team
  • Strong interest in value-based care and improving outcomes in vulnerable patient populations
