Qualifications

• New York NP or PA license and DEA (or eligible)
• Post-acute care experience required.  
• Minimum of 1 year primary care experience
• Background in geriatrics, hospital medicine, or emergency medicine also valued
• Strong clinical assessment and independent care-management skills
