Home Health RN Case Manager – Bay Area

In this role, you'll manage a patient caseload from admission through discharge, coordinating care across physicians, specialists, and referral partners to ensure seamless transitions from hospital to home. You'll develop and oversee individualized care plans, conduct skilled nursing visits, monitor patient progress, and serve as the central point of communication for the entire care team — all while building the kind of trusted relationships that make home health meaningful.

What You Bring: Active California RN license | Home health or case management experience preferred | Strong clinical assessment and communication skills | Commitment to patient-centered, compassionate care
