Physical Therapist - Home Health (San Francisco Bay Area - Positions open throughout "The Peninsula")

Position Summary:

The Physical Therapist provides skilled therapy services to patients in their home settings, focusing on restoring mobility, reducing pain, and maximizing physical function and independence. This role involves comprehensive evaluation, individualized treatment planning, hands-on intervention, and collaboration with patients, caregivers, and interdisciplinary healthcare teams to achieve functional goals.


Key Responsibilities:

Clinical Assessment & Treatment Planning

  • Perform comprehensive evaluations of patients' strength, range of motion, balance, gait, mobility, and functional limitations
  • Identify barriers to safe mobility, transfers, ambulation, and physical independence in the home environment
  • Develop patient-centered treatment plans based on clinical findings and physician referrals
  • Establish measurable, achievable goals in collaboration with patients and families

Direct Patient Care & Intervention

  • Deliver skilled therapeutic interventions including therapeutic exercise, manual therapy techniques, gait training, and balance activities
  • Provide instruction on safe transfer techniques, mobility strategies, and fall prevention
  • Recommend and train patients on assistive devices such as walkers, canes, and mobility aids to support safe independence
  • Address deficits in strength, endurance, flexibility, balance, coordination, and functional mobility

Education & Collaboration

  • Instruct patients, families, and caregivers on therapeutic exercise programs, safety strategies, equipment use, and methods to support functional progress
  • Coordinate care with physicians, nursing staff, and other rehabilitation professionals to ensure integrated service delivery
  • Participate in interdisciplinary case conferences and team meetings
  • Communicate regularly with referral sources regarding patient status and progress

Documentation & Compliance

  • Maintain accurate, timely documentation of all evaluations, treatments, and patient interactions
  • Ensure compliance with agency policies, state regulations, and federal healthcare guidelines
  • Follow payer-specific documentation and billing requirements
  • Participate in quality assurance and performance improvement activities

Professional Development

  • Maintain active professional licensure and required certifications
  • Complete mandatory continuing education and competency requirements
  • Stay current with evidence-based practice and clinical advancements


Qualifications:

Education & Licensure

  • Doctorate in Physical Therapy from an accredited program (or Bachelor's/Master's degree with equivalent experience)
  • Current, unrestricted Physical Therapist license in California
  • Current CPR/BLS certification

Experience

  • Minimum one year of clinical experience as a licensed Physical Therapist
  • Home health or rehabilitation experience strongly preferred

  • Excellent interpersonal and communication skills with diverse patient populations
  • Ability to establish therapeutic rapport and work compassionately with patients and families
  • Self-directed work style with strong organizational and time management capabilities
  • Proficiency with electronic medical record systems and digital documentation
  • Sound judgment in managing complex cases independently

Additional Requirements

  • Valid California driver's license and reliable personal vehicle
  • Ability to meet physical demands including patient transfers, equipment demonstration, and mobility assistance
  • Successful completion of background check and drug screening
