Registered Nurse - Home Health (San Francisco Area - Geographies throughout "The Peninsula")

Position Summary:

The Registered Nurse provides skilled nursing care to patients in their home settings, assessing health conditions, developing individualized care plans, and coordinating comprehensive patient care. This role involves clinical assessment, direct nursing interventions, patient and family education, and collaboration with physicians and interdisciplinary healthcare teams to promote optimal health outcomes and independence.


Key Responsibilities:

Clinical Assessment & Care Planning

  • Conduct comprehensive nursing assessments including physical, psychosocial, and environmental factors affecting patient health
  • Evaluate vital signs, symptoms, medication effects, and overall health status
  • Develop patient-centered care plans in collaboration with patients, families, physicians, and healthcare team members
  • Establish measurable goals and modify care plans based on patient response and changing needs

Direct Patient Care & Intervention

  • Provide skilled nursing interventions including wound care, medication management, IV therapy, catheter care, and ostomy management
  • Monitor patient conditions, identify changes in health status, and communicate significant findings to physicians and care team
  • Administer treatments and medications as ordered, ensuring safe and appropriate delivery
  • Implement infection control practices and promote patient safety in the home environment

Education & Collaboration

  • Educate patients and caregivers on disease management, medication regimens, symptom monitoring, and health promotion strategies
  • Coordinate care with rehabilitation therapists, social workers, and home health aides to ensure integrated service delivery
  • Supervise and provide clinical guidance to support staff in accordance with scope of practice and regulatory guidelines
  • Participate in interdisciplinary case conferences and team meetings
  • Communicate regularly with physicians and referral sources regarding patient progress and care needs

Documentation & Compliance

  • Maintain accurate, timely documentation of all assessments, interventions, and patient outcomes in electronic medical records
  • Ensure compliance with agency policies, professional nursing standards, state regulations, and federal healthcare guidelines
  • Follow payer-specific documentation and billing requirements
  • Participate in quality assurance and performance improvement activities

Professional Development

  • Maintain active professional licensure and required certifications
  • Complete mandatory continuing education and competency requirements
  • Stay current with evidence-based nursing practice and clinical advancements


Qualifications:

Education & Licensure

  • Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program; BSN preferred
  • Current, unrestricted Registered Nurse license in California
  • Current CPR/BLS certification

Experience

  • Minimum one year of clinical experience as a licensed Registered Nurse
  • Home health or acute care experience strongly preferred

Additional Requirements

  • Valid California driver's license and reliable personal vehicle
  • Ability to meet physical demands including patient transfers, equipment handling, and mobility assistance
  • Successful completion of background check and drug screening
