Occupational Therapist - Home Health (San Francisco Area - "The Peninsula")

Position Summary:
The Occupational Therapist provides skilled therapy services to patients in their home settings, focusing on restoring and maximizing independence in daily activities. This role involves comprehensive evaluation, individualized treatment planning, hands-on intervention, and collaboration with patients, caregivers, and interdisciplinary healthcare teams to achieve functional goals.
Key Responsibilities:
Clinical Assessment & Treatment Planning
- Perform comprehensive evaluations of patients' functional capabilities, cognitive status, and environmental factors affecting independence
- Identify barriers to performing self-care, household management, and meaningful daily activities
- Develop patient-centered treatment plans based on clinical findings and physician referrals
- Establish measurable, achievable goals in collaboration with patients and families
Direct Patient Care & Intervention
- Deliver skilled therapeutic interventions including functional training, fine motor activities, cognitive strategies, and physical reconditioning
- Provide instruction on adaptive techniques, safety strategies, and energy management
- Recommend and train patients on assistive devices and environmental modifications to support independence
- Address deficits in strength, coordination, endurance, cognition, and sensory-motor function
Education & Collaboration
- Instruct patients, families, and caregivers on therapeutic strategies, equipment use, fall prevention, and methods to support functional progress
- Coordinate care with physicians, nursing staff, and other rehabilitation professionals to ensure integrated service delivery
- Participate in interdisciplinary case conferences and team meetings
- Communicate regularly with referral sources regarding patient status and progress
Documentation & Compliance
- Maintain accurate, timely documentation of all evaluations, treatments, and patient interactions
- Ensure compliance with agency policies, state regulations, and federal healthcare guidelines
- Follow payer-specific documentation and billing requirements
- Participate in quality assurance and performance improvement activities
Professional Development
- Maintain active professional licensure and required certifications
- Complete mandatory continuing education and competency requirements
- Stay current with evidence-based practice and clinical advancements
Qualifications:
Education & Licensure
- Master's or Doctorate degree in Occupational Therapy from an accredited program
- Current, unrestricted Occupational Therapist license in California
- Current CPR/BLS certification
Experience
- Minimum one year of clinical experience as a licensed Occupational Therapist
- Home health or rehabilitation experience strongly preferred
