What You’ll Do
- Grow referral relationships with hospitals, SNFs, assisted/independent living facilities, physicians
- Educate providers and the community on in-home rehab and skilled care services
- Use market data (Trella) to target high-potential partners and drive smart outreach
- Coordinate closely with internal teams to ensure smooth patient onboarding
- Track activity and results to stay organized and ahead of goals
What You Bring
- 2+ years of experience in home health, hospice, or healthcare business development
- 1+ year in B2B sales is required with connections in the area
- A strong understanding of healthcare and referral networks
- Drive, confidence, and the ability to connect with people at every level
- Tech-comfortable and able to translate data into action
