Job Summary:

The Intake Coordinator will manage the initial intake process for new clients, ensuring that all necessary information is collected and documented accurately. This role involves interacting with clients, assessing their needs, and coordinating with other team members to facilitate smooth transitions into our services. The ideal candidate will have excellent communication skills, be highly organized, and possess a strong commitment to client care.


Responsibilities:

  • Client Intake: Serve as the primary contact for new clients, conducting initial assessments and gathering necessary information.
  • Documentation: Ensure accurate and complete documentation of client information in the system.
  • Coordination: Work closely with other team members to ensure a seamless transition for clients into our services.
  • Assessment: Evaluate client needs and determine eligibility for services.
  • Scheduling: Arrange appointments and follow-ups for clients with appropriate staff members.
  • Communication: Maintain regular communication with clients to provide updates and answer any questions.
  • Record Keeping: Maintain confidential client records in accordance with company policies and legal requirements.
  • Support: Provide administrative support to the team as needed.


Qualifications:

  • Experience: Previous experience in an intake or administrative role, preferably in a healthcare or social services setting.
  • Excellent verbal and written communication skills
  • Strong organizational and multitasking abilities
  • Proficiency in MS Office and familiarity with database management
  • Ability to handle sensitive information with discretion and confidentiality
  • Strong interpersonal skills and a compassionate approach
