Job Title: Primary Care Provider – Nurse Practitioner or Physician Assistant

Location: Dewey-Humboldt, AZ

Hours & Schedule: Full-Time

Work Environment: Outpatient Primary Care Clinic

Travel Required: No

Salary / Hourly Rate: $120k - $150k / year

Job Summary:

We are seeking a Primary Care Provider (NP or PA) to join our integrated healthcare team. This role focuses on providing high-quality, accessible care through well visits, same-day appointments, chronic disease management, and preventive health strategies. You will have the support of a medical scribe and will work closely with specialists to provide complete care to our patient population.


Job Duties & Responsibilities:

  • Conduct well visits, including Annual Physicals, Well Woman Exams, and Medicare AWVs
  • Manage same-day concerns such as fever, UTIs, headaches, and allergies
  • Treat and manage chronic conditions like diabetes, hypertension, and COPD
  • Promote preventive care, wellness, and healthy lifestyle education
  • Order and interpret diagnostic testing and labs as clinically appropriate
  • Follow clinic care flows and treatment guidelines consistently
  • Utilize EHR efficiently with timely and accurate documentation
  • Provide care for an average of 10–15 patients per day


Prerequisites / License & Certification Requirements:

  • Licensed Nurse Practitioner or Physician Assistant in the State of Arizona
  • Active, unrestricted DEA License (Arizona)
  • No history of malpractice claims
  • Eligible for credentialing with Medicare, Medicaid, and major commercial insurers
