Why work with us:

We deliver intensive outpatient therapy through trusted community health partnerships, ensuring patients receive care where they already feel supported. Our model provides world-class behavioral health care in collaboration with federally qualified health centers (FQHCs) to create a seamless, integrated patient experience. Our work is deeply mission-driven, focused on clinical excellence, care coordination, and improving the full patient journey.

What our ideal new team member looks like:

You are passionate about community-centered care, excited to partner with existing providers, and driven by an opportunity to influence a growing program. You value high-quality clinical work, evidence-based approaches, and meaningful patient connections. You want more than a traditional 8 to 5 — you want to help build something impactful.

Job Summary:

The Therapist will provide evidence-based behavioral health care within an intensive outpatient program (IOP) built in close partnership with local FQHCs. This role includes leading DBT-based group sessions, providing individual therapy, coordinating care with interdisciplinary teams, and supporting patient progress from intake through completion. The program primarily serves adults with Substance Use Disorder (SUD), many with co-occurring conditions, and will expand to adolescents in the future. Groups are mixed-gender with an emphasis on small, high-quality therapeutic environments.

Job Duties & Responsibilities:

  • Provide DBT-based individual and group therapy within the IOP model
  • Lead small group sessions capped at 12 participants, with program hours offered from 9:00 AM–12:00 PM and either 4:00–7:00 PM or 5:00–8:00 PM
  • Deliver compassionate, patient-centered care for a population primarily composed of Medicaid (60%), Medicare (10%), and Commercial (30%) members
  • Support individuals with SUD and co-occurring behavioral health conditions
  • Collaborate closely with FQHC care teams to ensure a fully integrated and coordinated care experience
  • Maintain accurate and timely documentation, monitor progress, and track treatment outcomes
  • Assist in refining the clinical curriculum as the program evolves and expands to additional populations
  • Foster strong therapeutic relationships within diverse and often bilingual patient groups
  • Support key performance indicators including quality, retention, completion rates, and patient satisfaction
  • Report to the Clinical Director in a model that prioritizes high-quality clinical care and a strong patient journey

Prerequisites / License & Certification Requirements:

  • Licensed behavioral health clinician (LCSW, LPC, LMFT, LCPC, or equivalent)
  • 4–5 years of behavioral health experience in outpatient or community-focused settings
  • Experience with both group facilitation and individual counseling
  • Ability to deliver trauma-informed, evidence-based care (DBT experience preferred)
  • Strong collaboration with interdisciplinary care teams
  • High emotional intelligence and commitment to patient-centered care
  • Bilingual or multilingual abilities, including Spanish, strongly preferred
  • Preferred: Experience with SUD, IOP/PHP programs, community health settings, CBT/DBT/MI/ACT modalities, and EHR systems
  • Detail-oriented, proactive, and motivated to contribute to a growing, mission-driven program

If you meet all of our criteria and would like to be considered, please apply with your most updated Resume/CV.
