Medical Case Manager Position

The Medical Case Manager position requires an individual who is organized, empathetic, and comfortable being a member of a team.

The Medical Case Manager enhances the quality of the client’s experience by leveraging their knowledge of the medical system and partnering with healthcare providers to ensure clients receive seamless, all-encompassing care, including:

• Educating and empowering clients and their family members to equip them with the resources and assistance they need to conquer their health challenges.
• Identifying and leading clients to community resources and the support they need to thrive.
• Coordinating client care with the assistance and direction of the physician.
• Assess treatment plans and keep impeccable client records (treatment plans, case notes, discharge plans, etc.). 
• Partnering with healthcare providers and fellow teammates to make sure every client gets the care they deserve. Attending important client appointments and advocate for them. 
• Assisting with hospital discharges as needed, assuring clients return to a safe and functional home environment.
• Establishing treatment programs by setting schedules and routines; coordinating services being provided; and arrange logistical details such as transportation. 
• Monitoring client’s rehabilitative progress by verifying attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements.

Desired Experience, Skills, and Abilities for the Medical Case Manager

• A degree in Nursing, Case Management, Physical Therapy, Speech and Language Pathology, Social Work, Respiratory Therapy, or Occupational Therapy is preferred
• 2+ years of experience in Microsoft Applications
• 1 or more years of experience in medical management, medical/team collaboration within a healthcare system
• Familiarity with HIPPA laws

This position is a flexible contract position, with a competitive compensation structure based on skills and experience.
