A growing Long-Term Care (LTC)–focused organization in Monsey is seeking a Licensed Pharmacist to support partner facilities with clinical pharmacy management and medication optimization. This role plays a key part in ensuring safe, effective, and compliant medication use across long-term care settings while collaborating closely with healthcare providers to improve patient outcomes.

The ideal candidate has hands-on LTC pharmacy experience, strong clinical judgment, and the ability to work both independently and collaboratively in a fast-paced healthcare environment.

Key Responsibilities:

• Manage Prior Authorizations, Formulary Exceptions, Step Therapy, and Quality Care Dosing initiatives
• Provide clinical guidance and recommendations on formulary and non-formulary medications
• Collaborate with physicians, nurses, and facility staff to optimize medication regimens and improve patient outcomes
• Conduct comprehensive chart reviews and document clinical interventions
• Support and participate in quality improvement initiatives across partner facilities
• Ensure compliance with LTC pharmacy regulations, best practices, and clinical guidelines
• Assist facilities with medication-related problem-solving and clinical decision-making
• Contribute to antimicrobial stewardship efforts and medication safety programs

Qualifications:

• Active Pharmacist license (required)
• Previous Long-Term Care (LTC) pharmacy experience (required)
• Strong knowledge of therapeutic interchanges, formulary management, and LTC medication protocols
• Excellent communication, documentation, and organizational skills
• Ability to work independently while collaborating effectively with interdisciplinary teams
• Strong attention to detail and clinical accuracy
• Antimicrobial Stewardship experience is a plus

Compensation: $130,000K-$180,000K/year
