RN Case Manager


The Case Manager is primarily responsible for incorporating marketing techniques, the admissions process and clinical assessment to promote and coordinate projects and activities resulting in the admission of appropriate residents to the facility while maintaining compliance with all applicable laws, regulatory and organizational standards.


Schedule: Monday - Friday 8:30am-5:00pm


Key Responsibilities:


  • Develops and executes strategies to build alliances with key referral sources such as doctors, hospitals and churches.
  • Distributes facility information and literature to referral sources and discharge planners.
  • Arranges and conducts tours of the facility.
  • Directs the inquiry and admissions process, including family and/or resident interviews, communication with referral sources, Exec Director and Director of Nursing regarding feasibility of admission and appropriate placement in facility as coordinated with nursing services.
  • Interviews resident or resident’s representative to obtain necessary personal and financial data to determine eligibility for admission.
  • Explains to the resident and/or guardian, room rates, billing procedures, visitors/guest privileges, restrictions, resident care procedures, etc.
  • Provides resident/family with information concerning resident funds, resident rights, advance directives, etc.
  • Prepares insurance assignment form with data such as names of insurance company and policyholder, policy number, and physician diagnosis.
  • Confers with nursing, housekeeping, transport and other staff members to coordinate and schedule admission of resident.
  • Prepares records of admission.



Position Requirements:

Required Education: Associate’s degree from an accredited two-year college, university or technical school

Preferred Education: Bachelor’s degree from an accredited four-year college or university

Required Experience: 5 years’ experience as RN, admissions, medical records, marketing, and/or social services or a combination of one or more
