We are a nonprofit dedicated to improving health equity and access to care for underserved communities across New York City. Through Community Health Worker-led programs, we help individuals and families connect to healthcare, social services, housing support, environmental health resources, and community-based care.
Position Summary
We are seeking a compassionate and highly organized Community Health Worker to support Medicaid members enrolled in the SIPPS and PHS Medicaid Care Programs under the New York State Medicaid 1115 Waiver.
In this role, you will serve as a trusted point of contact for participants, helping them navigate healthcare systems, access social services, address barriers to care, and stay connected to the support they need. This position is ideal for someone who enjoys working directly with communities, building relationships, solving problems, and making a meaningful impact in the lives of NYC residents.

This is a hybrid role that includes travel throughout New York City for participant visits, home visits, meetings, and community-based activities. Occasional evening or weekend flexibility may be needed based on participant and program needs.
