Position Summary:
As a Geriatric Nurse, you will focus on providing specialized care to elderly patients, addressing their unique healthcare needs and ensuring their well-being. Your role is crucial in helping elderly patients maintain their quality of life, manage chronic conditions, and receive the support they require for their physical and emotional health.
Key Responsibilities:
- Conduct comprehensive assessments of elderly patients to determine their healthcare needs, including physical, cognitive, and emotional aspects.
- Develop individualized care plans based on assessment findings, taking into account the specific needs and preferences of each patient.
- Administer medications, treatments, and procedures tailored to the elderly population, including those with chronic illnesses.
- Monitor and manage the overall health of elderly patients, including assessing changes in physical or cognitive health.
- Provide education to patients and their families on issues related to aging, chronic conditions, medication management, and safety in the home environment.
- Collaborate with the healthcare team, including physicians, social workers, and therapists, to ensure comprehensive care for elderly patients.
- Assess and manage pain, comfort, and end-of-life care for elderly patients, with empathy and compassion.
- Assist with activities of daily living (ADLs) for patients who may have mobility or self-care limitations.
- Maintain accurate and up-to-date patient records and ensure patient confidentiality.
- Stay current with best practices in geriatric nursing through continuing education and training.
- Contribute to quality improvement initiatives and the maintenance of a safe and effective healthcare environment for elderly patients.
Qualifications:
- Registered Nurse (RN) licensure.
- Bachelor of Science in Nursing (BSN) or higher preferred.
- Gerontological Nursing Certification (CGN) is a plus.
- Strong clinical assessment and critical thinking skills.
- Compassion, empathy, and effective communication with elderly patients and their families.
- Knowledge of common issues related to aging, such as dementia, diabetes, cardiovascular conditions, and arthritis.
- Proficiency in electronic medical record systems and healthcare software.
