Position Summary:
As a Nurse Case Manager, you will play a pivotal role in coordinating and managing the care of patients, ensuring they receive high-quality and cost-effective healthcare services. Your responsibilities will involve assessing patient needs, developing care plans, and collaborating with the healthcare team to promote optimal patient outcomes.
Key Responsibilities:
- Assess patient needs, medical conditions, and healthcare goals to create individualized care plans.
- Collaborate with physicians, specialists, and other healthcare professionals to coordinate the provision of medical services and treatments.
- Monitor patient progress, adherence to treatment plans, and the achievement of healthcare goals.
- Advocate for patients' rights and access to healthcare resources, including insurance coverage and community support services.
- Educate patients and their families on medical conditions, treatment options, and self-care strategies.
- Identify and address barriers to care, such as social determinants of health and access to transportation.
- Provide support and guidance to patients as they navigate the healthcare system, including assistance with appointments and referrals.

- Stay updated with industry trends and regulations, and contribute to quality improvement initiatives.
Qualifications:
- Registered Nurse (RN) licensure.
- Bachelor of Science in Nursing (BSN) or higher preferred.
- Case Management Certification (CCM or ACM) is a plus.
- Strong clinical assessment, communication, and problem-solving skills.
- Compassion, empathy, and the ability to build trust with patients and their families.
- Knowledge of healthcare systems, insurance coverage, and community resources.
- Proficiency in electronic medical record systems and healthcare software.
- Ability to work effectively within a multidisciplinary healthcare team.
