Position Summary:

As a Nurse Case Manager, you will play a pivotal role in coordinating and managing the care of patients, ensuring they receive high-quality and cost-effective healthcare services. Your responsibilities will involve assessing patient needs, developing care plans, and collaborating with the healthcare team to promote optimal patient outcomes.

Key Responsibilities:

• Assess patient needs, medical conditions, and healthcare goals to create individualized care plans.
• Collaborate with physicians, specialists, and other healthcare professionals to coordinate the provision of medical services and treatments.
• Monitor patient progress, adherence to treatment plans, and the achievement of healthcare goals.
• Advocate for patients' rights and access to healthcare resources, including insurance coverage and community support services.
• Educate patients and their families on medical conditions, treatment options, and self-care strategies.
• Identify and address barriers to care, such as social determinants of health and access to transportation.
• Provide support and guidance to patients as they navigate the healthcare system, including assistance with appointments and referrals.
• Ensure timely and accurate documentation of patient records, care plans, and interventions.
• Participate in regular case conferences and collaborate with the healthcare team to develop comprehensive care plans.
• Stay updated with industry trends and regulations, and contribute to quality improvement initiatives.

Qualifications:

• Registered Nurse (RN) licensure.
• Bachelor of Science in Nursing (BSN) or higher preferred.
• Case Management Certification (CCM or ACM) is a plus.
• Strong clinical assessment, communication, and problem-solving skills.
• Compassion, empathy, and the ability to build trust with patients and their families.
• Knowledge of healthcare systems, insurance coverage, and community resources.
• Proficiency in electronic medical record systems and healthcare software.
• Ability to work effectively within a multidisciplinary healthcare team.
