Additional Details about the position

• Provide comprehensive clinical pharmacy services for inpatient units, including medication order verification, therapeutic drug monitoring, and clinical consults.
• Review and interpret provider orders to ensure safe, evidence‑based, and cost‑effective medication use.
• Participate in daily interdisciplinary rounds and collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes.
• Prepare, dispense, and oversee the safe handling of medications, including sterile and non‑sterile products.
• Monitor medication therapy for efficacy, safety, interactions, and appropriate dosing, especially for high‑risk medications.
• Provide patient education on medication therapy, discharge prescriptions, and medication reconciliation.

EXPERIENCE REQUIREMENTS

• Hospital or clinical pharmacy experience preferred

REQUIREMENTS

• Education requirement: RPh or PharmD

LICENSURE REQUIREMENT

• Active OH License(or eligible for OH license)
