Utilizing a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care is central to our client's values, team culture, and mission.

Position Summary:

Under the direction of the Medical Director, the Primary Care Provider works in collaboration with our client's Physicians and the IDT. This role involves performing assessments, planning and providing primary care, and continuing medical education to participants.

Essential Job Duties:

  • Conduct in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments, and provide continuous medical coverage to participants.
  • Collaborate with physicians and the IDT to develop care plans for each participant.
  • Work with physicians and the IDT in discharge planning from acute or long-term settings.
  • Maintain the care of participants in nursing homes, making regular visits as needed.
  • Oversee and ensure effective care coordination in collaboration with physicians, community-based PACE physicians, and the IDT.
  • At the direction of the Medical Director, review outcomes and utilization data, including hospitalizations and specialist use, to establish best practices and policy improvements.
  • Participate in rotating night and weekend call duties.

Job Requirements:

  • Graduation from an Accredited School of Nursing.
  • Certification as a Nurse Practitioner with Prescriptive Authority.
  • NP State License.
  • Minimum of two years of experience working with the senior population.
  • Current knowledge of chronic care/geriatric issues and best practices.
  • Valid State Driver’s License with a clean DMV record.
  • Copy of recent Vehicle Insurance required.

Compensation:

  • Salary/Wage base range: $145,000 - $167,768.50 + Bonus + Equity.
  • Competitive total rewards package including 401k match, healthcare coverage, and a broad range of other benefits.
  • Actual pay will be adjusted based on experience and other qualifications.
