Job Overview:
A dedicated Physical Therapist is needed to join an interdisciplinary team (IDT) focused on providing high-quality, patient-centered care. This role is crucial in coordinating and delivering rehabilitative services to enhance participant independence and mobility, whether at the center or in-home settings.
Key Responsibilities:
- Conduct comprehensive Physical Therapy assessments in both facility and home environments.
- Develop and implement customized care plans in collaboration with an interdisciplinary team.
- Provide active and passive therapy interventions, including muscle re-education, gait training, and ADLs (Activities of Daily Living).
- Utilize treatment methods such as massage, heat, sound, water, light, or electrical therapy.
- Supervise Therapy Assistants, offering training and guidance for Functional Maintenance Programs (FMP).
- Educate staff, caregivers, and family members on therapeutic equipment usage and patient mobility strategies.
Qualifications:

Bachelor’s Degree in Physical Therapy with at least 5 years of experience.
- Master’s or Doctorate in Physical Therapy (Preferred).

Valid California Physical Therapy License or eligibility to obtain one.

Minimum of 2 years of experience in a clinical setting, preferably working with frail or elderly populations.
- CPR certification preferred.
- If educated outside the U.S., the Physical Therapy degree must be evaluated for U.S. equivalency.
Compensation & Benefits:

Base Salary: $100,000 - $120,000 (DOE)
