We are looking for Licensed Practical Nurses who are interested in a flexible side job. This per diem job allows you to customize your schedule and earn supplemental income while maintaining an autonomous relationship.

 

Position Overview: Licensed Practical Nurse - Health Risk Assessments

 

As an LPN, you will conduct in-home assessments for Medicare Advantage members in their homes. Each visit lasts approximately 20 minutes and involves utilizing evidence-based practices including point-of-care testing to identify diagnoses for care management.

 

Responsibilities of the Licensed Practical Nurse:


  • Willingness to see patients in one or more of the following areas:
  • Fairfield County - Stamford / Norwalk, Bridgeport / Stratford, Danbury, Hartford
  • Hartford County - Hartford
  • Litchfield County - New Milford / Terryville / Torrington
  • New Haven County - Waterbury, New Haven / Meriden
  • Middlesex County - Middletown
  • New London County - Norwich / New London / Groton
  • Tolland / Windham Counties - Stafford Springs / Willimantic
  • Utilize evidence-based practices and point-of-care testing to identify patient needs.
  • Ensure thorough documentation of diagnoses for care management.

 

Requirements of the Licensed Practical Nurse - Health Risk Assessments:


  • Active and unrestricted Licensed Practical Nurse license.
  • Minimum of 3 years of patient care experience (preferably in primary care, adult or geriatric settings and with EMR proficiency).
  • Minimum commitment of 18 hours per week, including one full 8-hour day (Sunday-Saturday).
  • Ability to work independently and autonomously.
  • Strong empathy, compassion, and interpersonal skills.
  • Excellent verbal and written communication abilities.
