Position Overview: Licensed Vocational Nurse - Health Risk Assessments

As an LVN, you will conduct in-home assessments for Medicare Advantage members in their homes. Each visit lasts approximately 20 minutes and involves utilizing evidence-based practices including point-of-care testing to identify diagnoses for care management.

Responsibilities of the Licensed Vocational Nurse:

• Willingness to see patients in the following Texas areas: Cypress, Katy, Waller and Prairie View.
• Utilize evidence-based practices and point-of-care testing to identify patient needs.
• Ensure thorough documentation of diagnoses for care management.

Requirements of the Licensed Vocational Nurse - Health Risk Assessments:

• Active and unrestricted Licensed Vocational Nurse license.
• Minimum of 3 years of patient care experience (preferably in primary care, adult or geriatric settings and with EMR proficiency).
• Minimum commitment of 18 hours per week, including one full 8-hour day (Sunday-Saturday).
• Ability to work independently and autonomously.
• Strong empathy, compassion, and interpersonal skills.
• Excellent verbal and written communication abilities.
