Nurse Practitioner/Physician Assistant, Wound Care
LOCATION: San Bernardino County / Riverside County / Los Angeles County; mobile/field-based within 1.5 hours of home; office visits (Ontario, CA) as required
EMPLOYMENT TYPE: Full-Time, W-2
COMPENSATION: $150,000–$170,000 (DOE), medical insurance, PTO, travel reimbursement per company policy

About the Job
The Nurse Practitioner or Physician Assistant will provide wound care services in a mobile capacity, traveling to patient homes and facilities across the assigned region. The role includes clinical assessment, procedures, treatment planning, and interdisciplinary collaboration.
What You’ll Do
● Deliver wound care services by visiting patients in homes and care facilities within your region
● Conduct thorough evaluations of acute and chronic wounds and determine appropriate treatment approaches
● Perform advanced wound care procedures, including sharp debridement and use of specialized products such as allografts
● Create and adjust individualized care plans that align with patient needs and healing progress
● Order and review laboratory tests and diagnostic studies to support clinical decision-making
● Accurately record all clinical interactions and treatment outcomes in the EMR system
● Work in partnership with physicians, home health nurses, and other members of the care team
● Track patient progress, modify care plans as necessary, and escalate concerns when appropriate
● Participate in team discussions and clinical updates to share insights and best practices
Qualifications
Required:
● Board-certified Nurse Practitioner (NP); Physician Assistant (PA) also considered
● Licensed in California
● Proficiency in wound care, with hands-on debridement experience
● Valid driver’s license and clean driving record (able to legally operate a motor vehicle)
● Willingness to travel within 1.5 hours of home, including occasional overnight stays
● Flexible and reliable; able to adapt to changing schedules and environments
● Proficiency with EMR systems and clinical documentation
Preferred:
● Strongly preferred: Wound Care Certified (WCC) and/or Certified Wound Specialist
● 1-3+ years of wound care experience
● Previous wound care/debridement experience (mobile setting not required)
● Strong preparation, communication, and clinical judgment skills
Reports to: Clinical Director
Works closely with: VP of Business Development, Medical Assistant(s)
