MDS Coordinator
Compensation: $140K-$170K pr/yr
Job Type: Full-Time and In-person position, Monday - Friday, Day shift.
Collaborating with a Long-Term Care/Rehabilitation Facility in Arverne, NY that is seeking a MDS Coordinator
MDS Coordinator Duties and Responsibilities:
- Oversees accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments (CAAs), and Care Plans per federal and state regulations.
- Acts as an in-house Case Manager, utilizing critical thinking and detailed knowledge of residents’ health to develop care pathways and communicate with residents, families, healthcare professionals, and payers.
- Collaborates with the Administrator and Director of Nursing to address regulatory risks, optimize resource capture on the MDS, and monitor clinical trends impacting care and outcomes.
- Works with therapy staff to select the appropriate assessment type, ARD, and RUG score, ensuring it reflects the care provided.
- Ensures timely MDS submission to the state database, reviews validation reports, and takes necessary follow-up actions.
- Facilitates Care Management, engaging residents, families, and the IDT to overcome discharge barriers and ensure smooth transitions.
- Educates and trains IDT members on the RAI process, including new or updated company policies. Analyzes QI/QM data with the Director of Nursing to identify trends and completes Utilization Review and Triple Check.

Qualifications & Requirements:
- Valid NYS RN License
- 2+ Year of Long-Term Care Experience is Required
- Prior experience as an MDS coordinator
