Role Overview

We are seeking a full-time Home Health Occupational Therapist to join our client's mission-driven team in Sacramento, CA. In this field-based role you will help patients regain independence in activities of daily living by delivering evidence-based OT interventions, adaptive-equipment training, and caregiver education.

You will self-schedule roughly 5–6 in-home visits per day, document care in the EMR, and collaborate closely with an interdisciplinary team—all while working in a supportive environment that values collaboration, patient-centered care, and professional growth.

The position pays $60 – $72 per hour (DOE), includes mileage reimbursement, and offers a full suite of medical, dental, vision, PTO, and 401(k) benefits. Open to relocating candidates who already hold an active California OT license

Key Responsibilities

• Evaluate functional abilities in the home
• Implement ADL-focused therapeutic activities (dressing, bathing, cooking)
• Recommend/adapt equipment for safety & independence
• Update care plans and document in EMR

Qualifications

• Active OT Therapist license in practicing state.
• OTD/MOT/BSOT degree
• Current CPR certification
• Reliable transportation with a valid driver’s license and auto insurance.
