Location: Glen Falls, NY

Compensation: $30.10 to $35.35 per hour + sign-on bonus.

Required Experience:

• RN or LPN license required.

Responsibilities:

• Perform comprehensive assessments of physical, mental, and emotional health to determine appropriate sub-accute care and rehabiliation.
• Develop and maintain positive relationships with area hospitals and community agencies
• Establish and maintain relationships with social workers, case managers, physicians and other contacts in the healthcare and payor community
• Educate residents and families about facility services, procedures, and care expectations
• Obtain insurance authorizations when needed.
