Community Health Worker (CHW)


Location: Santa Barbara, CA (Hybrid)


Classification: Non-Exempt


Compensation: $69,500–$101,000 annually, depending on experience



Overview:


Our client is seeking an empathetic and motivated Community Health Worker (CHW) to support members enrolled in a Dual Special Needs Program (D-SNP). As a trusted member of the care team, the CHW helps members and caregivers navigate healthcare and community resources, builds meaningful relationships, and removes barriers to care. This non-clinical role is ideal for someone passionate about community service and health equity.



Responsibilities:


  • Engage members and caregivers through phone, community, and home outreach to build trust and encourage participation in care plans.
  • Coordinate appointments, transportation, and follow-up activities to ensure members access needed services.
  • Conduct brief screenings for social needs such as housing, food, and transportation, escalating complex issues to licensed care team members.
  • Support transitions of care and encourage adherence to follow-up plans and medications.
  • Provide health education using simple, clear communication and teach-back methods.
  • Maintain accurate documentation of interactions, referrals, and outcomes in the care management system.
  • Participate in team meetings and maintain collaboration with RN Care Managers, Social Workers, and community partners.
  • Uphold confidentiality, professionalism, and cultural sensitivity at all times.



Qualifications:


  • High school diploma or GED required.
  • 2–3 years of experience in a community-facing or member-support role (outreach, navigation, social services, or similar).
  • Familiarity with community resources such as IHSS, Medi-Cal, housing, food programs, and transportation services.
  • Strong interpersonal skills and ability to build rapport with diverse populations.
  • Excellent communication, organization, and follow-up skills.



Preferred:


  • CHW certificate or willingness to complete approved training within two years of hire.
  • Bilingual in Spanish.



Location & Work Environment:


Hybrid role with some in-person community and home visits required. Regular business hours with flexibility for outreach needs.
