The Supportive Services Case manager fosters a positive and inclusive living environment for tenants with Severe mental health conditions. This role involves providing case management emotional support and ensuring effective communication between tenants, property management, and relevant stakeholders. The goal is for individuals living with Severe mental illness to be independent and maintain a good quality of life.
Responsibilities
Case Management: Conduct individual assessments to identify specific needs and challenges tenants with mental health diagnoses face. Collaborate with mental health professionals to
understand treatment plans. Utilize goal-setting and positive coping strategies to help tenants live their best life.
Community Building: Establish positive relationships within two apartment complexes. Working with each other to develop community and positive communication. The overall goal is to decrease isolation.
Communication and Coordination: Serve as a liaison between tenants, property management, internal program staff, mental health providers, and community providers. Communicate effectively with all stakeholders to ensure a holistic approach to tenant well-being.
