Nurses are not expected to cover more than 1 borough although they are welcome to. Whether you’re looking for a full-time, part-time, or per diem position, we have options to fit your needs.

As a Field RN Supervisor, you’ll play a vital role in delivering professional nursing services while adhering to governmental regulations, State Nursing Practice Acts, and the home care company’s values and procedures. You’ll create and oversee individualized Plans of Care, supporting caregivers to ensure our patients enjoy the best possible health outcomes.

Qualifications:

 **Educational:** R.N. Diploma from an NLN-approved Nursing Program

**Experience:** At least one year of home health experience

 **Licensure:** Must meet all state and federal requirements for licensure and employment

Additional Requirements:

- Familiarity with Joint Commission/CAHC/DOH regulatory requirements

- Strong understanding of nursing principles and their application in home care

- Ability to work independently with minimal supervision

- Reliable transportation to visit patients across at least one borough

Essential Job Responsibilities:

- Conduct initial patient assessments for those newly admitted to our care

- Develop personalized, goal-oriented Plans of Care

- Supervise and provide care as outlined in the Plan of Care

- Oversee and guide Home Health Aides (HHAs)

- Create and update the Aide Plan of Care based on initial assessments

- Evaluate the effectiveness of caregivers

- Ensure accurate and timely documentation of client care and supervisory visits
