Physical Therapist (PT) – Certified Home Health Agency

As a Physical Therapist, you will provide one-on-one care to patients in their homes, helping them improve mobility, strength, independence, and quality of life. You will be assigned to one borough only (Brooklyn, Bronx, or Queens), based on your preference.

No cross-borough travel is required.

Mileage reimbursement is offered for all home visits.

About the Role

We welcome experienced PTs as well as new graduates interested in home care. Full training and support are provided.

Key Responsibilities

• Perform home-based PT evaluations and treatments
• Develop and implement individualized treatment plans
• Educate patients and families on therapeutic exercises and safety
• Maintain professional and timely clinical documentation
• Manage caseload and schedule within your assigned borough

Qualifications

• Active New York State Physical Therapist (PT) license required
• Strong communication and clinical assessment skills
• Ability to work independently in the community
