đ©ș The Role: Nurse Manager / Wellness Nurse
The Nurse Manager / Wellness Nurse plays a key leadership role, overseeing clinical care delivery within assigned neighborhoods while partnering closely with the Director of Nursing and interdisciplinary leadership team. Youâll guide nursing and caregiving staff, ensure regulatory compliance, and uphold the highest standards of resident careâall within a collaborative, well-resourced environment.
đ Key Responsibilities
Leadership & Team Support
- Supervise and mentor nursing staff; provide administrative oversight when the Director of Nursing is unavailable
- Coach, train, and develop staff through consistent communication and hands-on leadership
Resident Care & Clinical Oversight
- Participate in pre-admission evaluations
- Develop and oversee Individualized Service Plans (ISPs)
- Conduct ongoing assessments and reassessments, adjusting care plans as needed
- Coordinate care with physicians, pharmacy, home health, hospice, and rehab providers
- Partner with Neighborhood Managers to proactively address resident needs and quality outcomes
- Oversee medication ordering, storage, administration, and documentation
Quality & Compliance
- Participate in Quality Assurance initiatives and performance improvement efforts
- Ensure timely, accurate clinical documentation
Family Communication
- Collaborate with Neighborhood Managers to maintain open, supportive communication with residentsâ families

Qualifications
- Active RN license in New York State

3+ years of leadership or supervisory experience in senior living, assisted living, home care, or related healthcare setting
- Skilled Nursing experience is appreciatedÂ
- Memory Care experience strongly preferred
- Familiarity with ISPs, 3122 forms, and high-acuity resident care a plus
- Strong leadership, communication, and interpersonal skills
