RN Case Manager – CHHA | Office-Based | Brooklyn, NY

We’re seeking an experienced RN Case Manager to manage clinical coordination, review documentation, and support field staff in delivering exceptional care. This is a Monday–Friday, office-based role located in Brooklyn, offering stability, growth, and the opportunity to be part of a respected, mission-driven team.

Key Responsibilities

• Coordinate and manage all aspects of assigned patient cases in compliance with CHHA and state standards.
• Review and approve assessments, care plans, and clinical documentation for accuracy and completeness.
• Serve as a resource for field nurses and interdisciplinary team members to ensure quality and continuity of care.
• Communicate effectively with physicians, therapists, and caregivers to support timely and coordinated care.
• Monitor visit frequencies, authorizations, and documentation to support regulatory and billing compliance.

Qualifications

• Active NY RN License (required)
• CHHA or home health experience (required)
• Strong understanding of home care compliance and OASIS documentation
• Excellent communication, organization, and follow-up skills
• Ability to work independently and collaboratively in a fast-paced environment
• Experience with EMR systems preferred
