Chief Operating Officer — Home Care Division

Seeking an experienced, skilled home care operator to lead a newly launched home care line of business within a well-established and growing healthcare organization.

The Role

This is not a management role inside someone else's structure. This is a true builder's seat — with real authority to shape and lead the operational infrastructure of this division from day one, reporting directly to the CEO.

This division operates as a brokerage model: connecting home care agency partners with the organization's existing Medicaid peer support contracts, enabling home care staff to serve as Certified Peer Specialists and bill under those contracts. The COO will be the operational architect of this model — building the systems, relationships, and processes that make it work at scale.

What You'll Own

• Day-to-day operations of the home care division across two offices
• Relationships with home care agency partners — building, maintaining, and managing them
• Managing and negotiating MLTC contracts, including rate discussions, contract terms, and payer relationships
• Compliance oversight: DOH, Medicaid billing, documentation, EVV, and authorization management
• Establishing and overseeing the process for vetting and onboarding staff serving as Certified Peer Specialists under the organization's Medicaid contract
• Billing accuracy and session documentation across partner agencies
• Operational systems, workflows, and policies that will scale as the program grows
• Serving as the senior operational voice between the CEO and the agency partner network

What We're Looking For

• A seasoned home care operator — LHCSA, CDPAP, or home health background
• Strong command of Medicaid billing, MLTC contracting, and DOH compliance
• Experience managing multi-site or multi-program operations
• Executive presence — polished, direct, and comfortable at the leadership table
• A compliance mindset paired with the ability to build structure in a fast-moving environment
• Someone who has launched or grown programs before and knows what that actually takes

Compensation & Benefits

This position offers an executive-level compensation and benefits package, including base salary, performance-based bonus, medical, dental, and vision insurance, generous PTO, and 401(k).
