Family Medicine Physician

Patients stay healthier when one physician owns the relationship, the plan, and the follow-through. In this role you will be the clinical home for a panel of families—preventing illness, managing chronic disease, and making timely decisions that keep people out of the hospital. Success is measured in access, outcomes, and trust, not volume for its own sake.

Performance Objectives

  • Build and retain a balanced panel by delivering same- or next-day access for urgent needs and scheduled preventive care that meets evidence-based guidelines.
  • Close care gaps for diabetes, hypertension, and preventive screenings so quality metrics improve quarter over quarter.
  • Reduce avoidable ED and hospital use by managing acute issues in clinic and coordinating timely specialty and behavioral-health follow-up.
  • Document complete, defensible notes the same day so coding, quality reporting, and continuity are accurate without after-hours backlog.
  • Lead the care team—nurses, MAs, and care coordinators—so in-basket, refills, and results are handled within agreed turnaround times.
  • Earn strong patient experience scores by explaining plans clearly and involving patients in decisions they can actually follow.

Environment & Resources

You will practice in a collaborative outpatient primary-care setting with nursing and care-coordination support, a modern EHR, and defined protocols for chronic disease and transitions of care. You report to the medical director, with protected time for panel management and peer review. Compensation reflects panel responsibility and quality, not only encounters.

Essential Qualifications

  • MD or DO and board certification or board eligibility in Family Medicine.
  • Active, unrestricted state medical license and DEA registration, or eligibility to obtain both before start.
  • Demonstrated outpatient continuity practice managing a full spectrum of acute, chronic, and preventive care across the lifespan.
  • Proven ability to meet quality, access, and documentation standards in an EHR-based clinic.
  • Skill leading a care team and communicating clearly with patients and specialists.

Role Selling Points

  • A true medical home: you own the relationship, not a shift.
  • Team support and protocols so clinical judgment is not buried in administrative work.
  • Quality and access goals that are visible, fair, and tied to how you are recognized.
  • A practice designed for long-term physician partnership, not short-term coverage.

If you want a panel you can be proud of and the support to care for it well, apply with your CV and a brief note on the outcomes you have improved.
