Geriatrics Physician Assistant

Older adults do not need more appointments. They need a clinician who can see the whole picture—function, medications, cognition, goals, and the family holding it together—and act before a small problem becomes a crisis. In this role you will own day-to-day medical decisions for complex older patients and turn fragmented care into a plan people can actually live with. Your judgment will keep more patients safer at home, clearer about their choices, and out of avoidable hospital stays.

Performance Objectives

  • Complete comprehensive geriatric assessments and deliver care plans that address medical, functional, cognitive, and social risks within the first visits.
  • Manage multimorbidity and polypharmacy so each regimen is simpler, safer, and aligned with the patient’s goals—including timely deprescribing.
  • Reduce preventable acute utilization by catching decompensation early and coordinating swift follow-up after ED or hospital events.
  • Lead goals-of-care and advance-care-planning conversations that produce documented decisions families and the care team can follow.
  • Close high-risk gaps—falls, delirium, dementia progression, nutrition, and caregiver strain—with measurable follow-through.
  • Direct an interdisciplinary plan with nursing, pharmacy, therapy, and social work so handoffs are clear and nothing critical is dropped.
  • Document to a standard that supports quality review, billing integrity, and continuity for every clinician who sees the patient next.

Environment & Resources

You will practice within a physician-led geriatric team, with a defined panel and reliable access to nursing, pharmacy, therapy, and care coordination. Day-to-day tools include the electronic health record, evidence-based geriatric protocols, and a supervising physician for collaboration on complex cases. You will have the clinical authority to assess, treat, prescribe within collaborative agreement, and escalate when the plan needs a different level of care.

Essential Qualifications

  • Graduate of an accredited PA program and current NCCPA certification.
  • Active state PA license and prescriptive authority, including DEA registration where required for the practice.
  • Demonstrated experience managing older adults with multimorbidity, cognitive impairment, or post-acute needs.
  • Skill in geriatric assessment, medication review, and goals-of-care discussions—not only disease-by-disease treatment.
  • Ability to prioritize, document clearly, and coordinate across disciplines without losing clinical ownership.
  • Comfort practicing with supervised autonomy and knowing when to escalate.
