POSITION TITLE:         Clinical Pharmacist 

PROGRAM:                   Post Concussion Management Program  

REPORTS TO:              Rehabilitation Services Manager - PCMP

LOCATION:                    Newton, Surrey

STATUS:                        Part Time, Permanent Employee Status (0.4 FTE | 15 hours/week) or Contractor Status

HOURS OF WORK:       15 hours/week | 2 days/week

Wage Range: $43.00 to $53.00 /hour (Employee Rates)

Contractor Rate: TBD



 

RESPONSIBILITIES

•      Optimize medication treatment plans to minimize actual and potential adverse effects while maximizing benefits, supporting the best possible outcomes for clients’ functional ability and return to work

•      Provide evidence-based recommendations and clinical insights to the team Physician and clients, particularly in complex cases, with a focus on improving overall health

•      Educate and support clients in safely managing medications at work and home, promoting adherence and ensuring suitability for safety-sensitive roles

•      Collaborate with clients and the treatment team to assess how medication changes impact functioning

•      Review and assess medications to identify opportunities for optimization, with a focus on pain, headaches, mood, and sleep

•      Reduce risks associated with current and potential treatments

•      Educate clients to support understanding, cooperation, and adherence to medication management plans

•      Work closely with the team Physician to develop and implement treatment plans, including titrations, tapers/weaning, and withdrawal management

•      Provide ongoing weekly monitoring and reassessment to adjust plans, support functional goals, and enhance overall quality of life

 

 

QUALIFICATIONS

  • Licensed Pharmacist in good standing with the College of Pharmacists of British Columbia
  • Minimum of 2 years of experience working with patients with concussions or in a similar clinical setting
  • Strong clinical knowledge with a commitment to patient-centered care
  • Excellent communication, problem-solving, and organizational skills
  • Ability to work independently and make sound clinical and operational decisions
  • Comfortable working within a similar work setting such as the post-concussion, pain management or return to work program with a broad scope of responsibilities
