Occupational Therapist - Illinois (In-Home Services)


Position Overview

The Occupational Therapist will provide in-home therapy services to patients as prescribed by the attending physician. This role involves assessing patient needs, developing and implementing treatment plans, and offering consultation to patients, families, and healthcare teams. The goal is to improve and restore patient independence and physical/cognitive functioning through therapeutic interventions and education.


Key Responsibilities

  • Perform assessments of patient functionality and develop short- and long-term goals to improve independence and physical/cognitive abilities.
  • Design and implement approved treatment plans tailored to the patient’s needs.
  • Educate patients and family members on therapeutic, creative, and self-care techniques, and train them in using orthotic or prosthetic devices as necessary.
  • Conduct re-evaluations of patient progress and revise treatment plans accordingly.
  • Maintain accurate clinical and progress notes, submitting paperwork promptly as per agency policy.
  • Provide periodic updates to the attending physician on patient progress and recommend necessary changes in treatment plans.
  • Arrange for outpatient occupational therapy services if treatments cannot be provided in-home.
  • Offer consultation and training to other home health team members on equipment, techniques, and modalities when needed.
  • Participate in in-service training, staff meetings, and agency development activities.


Minimum Requirements

  • Bachelor’s degree or higher in Occupational Therapy.
  • Valid Occupational Therapist, Registered (OTR) license in Illinois.
  • Minimum 1 year of experience in Occupational Medicine.
  • Minimum 2 years of experience in Social Work.
  • Valid driver’s license (DL).
  • Certification in one of the following:
  • American Heart Association Basic Life Support (AHA BLS).
  • American Heart Association Advanced Cardiac Life Support (AHA ACLS).
  • American Heart Association Pediatric Advanced Life Support (AHA PALS).


Schedule and Travel

  • Flexible work schedule based on assigned caseload.
  • Local travel is required to patients' homes within the service area.
