We are seeking a highly skilled and compassionate Director of Case Management to lead and enhance patient care coordination and management at Valley View Medical Center. This role is pivotal in aligning our strategic goals with the effective and efficient management of patient needs, discharge planning, and resource utilization. The ideal candidate will bring a background in nursing, case management, and critical care, ensuring continuity of care and promoting cost-effective healthcare delivery that enhances patient satisfaction.
Key Responsibilities:
- Lead comprehensive case management, including patient assessment, planning, and coordination from admission through discharge
- Oversee and integrate social services, case management functions, and discharge planning with other departments and external agencies
- Conduct regular medical record reviews for resource utilization and compliance with standards (TJC, CMS, etc.)
- Serve as a patient advocate, providing staff education on discharge planning and psychosocial aspects of care
- Collaborate with physicians to maintain desired patient outcomes, cost per case, and manage clinical resources effectively
- Facilitate interdisciplinary rounds and conferences to review treatment goals, optimize resources, and address post-hospital needs
- Develop, implement, and direct patient care policies and protocols to address special cases and patient needs
- Evaluate and monitor patient satisfaction and quality of care, addressing any issues proactively
- Act as a point of contact for patients and families, providing clear communication and support throughout the care continuum
Leadership Responsibilities:

Human Resources Management: Recruit, train, and manage case management staff, fostering a supportive and effective team environment

Financial Management: Monitor budget, cost per case, and financial resources to ensure cost-effective care

Performance Improvement/Safety: Implement and monitor safety protocols, continuously seeking to improve patient outcomes and quality of care
Qualifications (Must-Haves):

Education: Bachelor of Science in Nursing (or higher)

Licensure: Arizona Registered Nurse (RN) or Compact State RN licensure

Certifications: BLS certification upon hire

Experience: Minimum of 2 years as a Director of Case Management in a critical care hospital
Preferred Qualifications (Nice-to-Haves):
- Certification in Case Management (CCM) or Accredited Case Manager (ACM)
- Master’s degree in a related field
- 5+ years of leadership experience in case management or critical care hospital management
Skills & Abilities:
- Excellent communication and interpersonal skills to build relationships with patients, families, and healthcare teams
- Strong problem-solving abilities and critical thinking skills for managing complex patient cases and achieving desired clinical outcomes
- Proficiency in case management practices, discharge planning, and resource utilization management
- Familiarity with regulatory standards and compliance requirements in healthcare settings
