Physical Therapist - Franklin, PA

Position Overview

Join a compassionate and collaborative care team dedicated to delivering outstanding home health services. This role involves managing and delivering patient care plans in the Franklin, Ford City, and surrounding areas. Key Responsibilities

• Develop and manage patient care plans to improve mobility, reduce pain, and prevent further injury.
• Provide home-based physical therapy services while collaborating with the healthcare team.
• Conduct evaluations, develop treatment plans, and adjust as necessary to meet patient goals.
• Maintain accurate and timely documentation using EMR systems.
• Educate patients and families about physical therapy techniques, exercises, and equipment usage.
• Drive to and from patient locations, ensuring care is provided promptly.

Minimum Requirements

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Valid Physical Therapist license in Pennsylvania.
• At least 1 year of experience in healthcare.
• Proficiency with EMR systems, including Practice Partner or other platforms.
• Valid driver’s license with the ability to travel locally (~20% of workday).
• Strong time-management skills and ability to self-schedule effectively.

Schedule and Shift Details

• Work Hours: Monday–Friday, 8:00 AM–5:00 PM (light full-time available, 4 days/week).
• On-Call/Weekends: 1 weekend/month and 2 holidays/year.
• Travel: Local travel to patient homes in the service area.
