Home Health Physical Therapist

Join our client’s dedicated home health team as a skilled physical therapist, and you’ll provide essential therapy services directly in patients' homes. In this role, you’ll help patients regain independence and improve physical and cognitive functioning by developing and implementing personalized treatment plans. This position includes comprehensive patient and caregiver education, regular progress updates, and collaborative work with other healthcare professionals.

Key Responsibilities:

  • Perform Physical Therapy assessments and create individualized treatment programs tailored to each patient's needs.
  • Record and report on patients’ progress, adjusting treatment plans to achieve therapeutic goals.
  • Educate patients and caregivers on therapy routines and safe equipment usage to foster ongoing health improvements.
  • Supervise Physical Therapy Assistants and, when necessary, Certified Nursing Assistants, ensuring high-quality care delivery.
  • Prepare detailed reports and actively participate in staff meetings and development activities within the Agency.

Qualifications:

  • Education: Master’s degree or higher in Physical Therapy.
  • Experience: Minimum 1 year of experience as a Physical Therapist (Home Health experience preferred but not required; inpatient rehab experience may substitute).
  • Licensure: Must hold a valid Physical Therapist license in Illinois (IL).
  • Other Requirements: Valid driver’s license (DL) and willingness to travel locally to patients’ homes.
  • Preferred Skills: Experience with OASIS and HCHB (training provided if needed).
