Population Health & Value-Based Care SME (Healthcare)

Why You’ll Actually Care

District Partners is exclusively supporting a rapidly growing, investor-backed healthcare SaaS company where deep population health expertise directly shapes the technology.

The platform helps provider organizations turn complex clinical, financial, and operational data into better quality outcomes, stronger care coordination, and improved performance under value-based reimbursement models.

This opportunity is built for a senior, hands-on Population Health / Value-Based Care SME who understands how care teams manage populations, prioritize risk, close care gaps, and improve quality performance. You’ll translate that expertise into product requirements and workflows that address real provider challenges.

The Basics

This is a highly visible, strategic individual contributor role working directly with executive leadership, Product, Engineering, Implementation, and Customer Success.

You’ll bring the domain expertise to define what the platform needs to accomplish, explain why it matters, and guide how those requirements translate into practical capabilities.

What You’ll Be Owning

  • Translate population health, care management, risk stratification, and care gap processes into clear product requirements and scalable workflows
  • Define user needs, decision points, data requirements, and expected outcomes so Product and Engineering can build capabilities that reflect how providers work
  • Apply hands-on MIPS and healthcare quality expertise to product decisions, reporting requirements, and customer challenges
  • Inform roadmap priorities through your understanding of provider needs, reimbursement models, regulatory requirements, and quality initiatives
  • Help teams interpret how clinical, operational, quality, financial, and claims data support population health decisions and value-based performance
  • Work with executive leadership and customers to clarify complex domain questions and recommend practical solutions
  • Support Implementation and Customer Success with workflow alignment, adoption, and subject matter guidance

The Environment

The company needs a senior domain expert who has remained close to the actual work. You should be comfortable explaining how populations are segmented by risk, how care teams prioritize interventions, and how those activities connect to quality performance and reimbursement.

Success requires moving from those discussions into specific, actionable direction for product and technical teams. Healthcare data fluency matters, alongside the broader population health judgment to interpret what that data means for care delivery and platform functionality.

Leadership experience is welcome, provided you can personally work through the details and bring current, hands-on expertise.

What You Bring to the Table

  • Deep experience in population health and value-based care, including provider operations, care management, care coordination, quality performance, and reimbursement
  • Practical expertise in risk stratification, population segmentation, and care gap identification and closure
  • Hands-on MIPS expertise is required, including the ability to personally navigate measures, reporting requirements, provider workflows, and performance challenges, and translate that expertise into actionable product requirements
  • Strong knowledge of HEDIS, CMS/Stars, NCQA, and related healthcare quality measurement and reporting frameworks
  • Understanding of how clinical, quality, operational, and claims data inform population health workflows and value-based performance
  • Experience with health systems, physician groups, ACOs, CINs, health plans, or other organizations managing population health and value-based care
  • Experience with population health platforms, EHRs, care management solutions, analytics platforms, or related enterprise healthcare applications
  • Demonstrated ability to translate complex healthcare needs into product requirements, workflow definitions, and practical guidance for Product and Engineering
  • Senior-level judgment and clear communication across clinical, operational, technical, and executive audiences

A Few Things Worth Knowing

  • Compensation: $150,000 to $200,000 base salary, plus bonus and comprehensive benefits. Compensation will reflect experience, expertise, and overall alignment.
