The Health Center Association of Nebraska (HCAN) is actively recruiting a School-Based Licensed Mental Health Practitioner to join our Federally Qualified Health Center (FQHC) in Norfolk, Nebraska, where you will provide high-quality, patient-centered behavioral health services to students and patients. Utilizing an integrated care approach, our providers work collaboratively to address all aspects of patients’ health and believe that exceptional, compassionate care should be accessible and affordable for all. Medical, dental, behavioral health, and outreach services are coordinated to ensure well-rounded, comprehensive care.

Your Role:
- Provide direct mental and behavioral health services to students in local schools
- Provide comprehensive assessment and diagnosis using specialized clinical knowledge and advanced clinical skills
- Develop comprehensive, individualized treatment plans and goals for the patient
- Collaborate with other members of the multidisciplinary team, as appropriate, to develop treatment goals for the patient
- Complete necessary documentation and reporting in accordance with departmental standards and Federal and State regulations
- Consult and cooperate with community systems to facilitate crisis management, advocacy, referral, and follow-up with the focus on attaining treatment goals
What We Are Looking For:
- Master’s degree from an accredited college or university in a mental health related field
- Current license in good standing as a Mental Health Practitioner in the state of Nebraska (PLMHP, LMHP, LIMHP)
- Knowledge of signs and symptoms of mental health disorders
- Demonstrated clinical decision-making skills appropriate to client care and safety
- Experience in community-based behavioral health care and case management
- Training in crisis intervention
- Must have a valid driver’s license and the ability to utilize own transportation for work purposes
