Position: Telehealth Nurse (LPN) – On-Site, Bilingual Spanish

Location: Long Island, NY

Our client is a rapidly growing medical practice dedicated to providing high-quality primary care services to the Long Island community. Their services include preventive care, routine physicals, chronic disease management, and more. To support continued growth and improve patient outcomes, they are seeking a compassionate and organized Licensed Practical Nurse (LPN) to join their team. This role focuses heavily on patient follow-up via phone calls to ensure continuity of care and promote positive health outcomes.


Bilingual Spanish proficiency is REQUIRED for this position.


Key Responsibilities:

  • Conduct daily follow-up phone calls with patients following their clinical visits.
  • Use a standardized script to assess:
  • Whether patient concerns were fully addressed during their appointment.
  • Patient suggestions for improving their care experience.
  • Current medication adherence, new prescriptions, and any side effects or challenges.
  • Verify whether necessary follow-up appointments have been scheduled.
  • Identify and document potential barriers to care, including:
  • Access to medications, transportation issues, or difficulty scheduling appointments.
  • Emotional or psychological concerns such as anxiety around medical visits.
  • Lack of access to healthy food or understanding of medical conditions/treatments.
  • Perform brief symptom checks (e.g., pain, dizziness, nausea, shortness of breath).
  • Accurately document all patient interactions in the Electronic Medical Record (EMR).
  • Conduct depression screenings when appropriate and escalate concerns to the care team.
  • Confirm patient understanding of care plans and medication instructions.
  • Collaborate with providers and clinical staff to ensure all patient needs are addressed.



Requirements:

  • Active and unrestricted LPN license in the State of New York.
  • Graduate of an accredited Licensed Practical Nursing program.
  • At least 1–2 years of clinical experience preferred, ideally in outpatient or primary care settings.
  • Strong clinical judgment and patient communication skills.
  • Proficiency with Electronic Medical Records (EMR).
  • Excellent interpersonal, verbal, and written communication abilities.
  • Team-oriented with a compassionate, patient-focused approach.
  • Organized, dependable, and comfortable in a fast-paced environment.
  • Fluent in Spanish (required).
