Position Overview
We are seeking a passionate, relationship-driven Hospice Liaison to join our team and represent our client in the greater Atlanta or Newnan area. This is a field-based, full-time sales role ideal for a results-oriented professional with an established referral network and a strong track record in hospice or home health business development.
As a Hospice Liaison, you will serve as the face of our organization to healthcare providers, patients, and their families. Your primary responsibility will be to build and nurture referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare partners. You will also play a vital role in patient admissions by conducting assessments, educating families about services, and ensuring a smooth transition into hospice care.

Key Responsibilities
- Business Development & Relationship Management
- Cultivate and maintain strong relationships with referral sources including physicians, discharge planners, case managers, hospital systems, SNFs, and ALFs within the assigned territory
- Conduct regular in-person visits to referral partners to maintain visibility and trust
- Identify and pursue new referral opportunities through networking, community events, and territory intelligence
- Community Education & Outreach
- Serve as a knowledgeable and compassionate educator on hospice and palliative care services
- Conduct presentations, in-services, and educational events for medical professionals and community organizations
- Act as a brand ambassador to increase awareness and trust in Blue Summit’s services
- Patient & Family Support
- Meet with patients and families to provide information, answer questions, and guide them through the hospice admission process
- Coordinate with clinical intake and care teams to ensure seamless onboarding and delivery of services
- Advocate for the patient’s needs while supporting their emotional and psychosocial well-being
- Territory Strategy & Reporting
- Maintain a structured schedule of field visits and engagement activities
- Track referrals, admissions, and outcomes to evaluate effectiveness and inform strategy
- Collaborate closely with leadership and interdisciplinary teams to align goals and improve outcomes
Qualifications
- Required Experience
- Minimum 2 years of experience in hospice or home health sales within the Atlanta or Newnan area
- Demonstrated success in growing and managing a referral network
- Proven ability to develop business relationships and drive census growth
- Clinical & Industry Knowledge
- Solid understanding of Hospice Care, Palliative Care, and End-of-Life Care philosophies and practices
- Ability to explain medical and care-related topics in a clear, compassionate manner
- Skills & Attributes

- High emotional intelligence and empathy for patients and families
- Self-starter with a strong sense of accountability and purpose
- Organized, detail-oriented, and capable of managing multiple relationships and priorities simultaneously
- Comfort working independently in the field and collaboratively with an interdisciplinary care team
- Other Requirements
- Valid driver’s license, reliable transportation, and willingness to travel throughout assigned territory
- Must reside in or near the greater Atlanta or Newnan area
- Existing book of business/referral relationships in the territory is required
