Job Title: Clinical Lead Nurse
Reporting to: Registered manager
Hours per week: 08.00-17.30
Location: Remote
Aim of the role
The Clinical Nurse is responsible for overseeing the delivery of high-quality patient care, ensuring compliance with clinical standards, and managing a team of community nurses. This role involves reviewing care plans and risk assessments, attending multidisciplinary team (MDT) meetings, monitoring medication audits, and ensuring that all nursing interventions are carried out effectively.
Key responsibilities will encompass:
1. Clinical Leadership
· Supervise and manage ad hoc community nurses, ensuring high standards of patient care.
· Conduct regular reviews of care plans and risk assessments, ensuring they are up to date and reflective of patient needs.
· Oversee and evaluate the work of community nurses, providing feedback, support, and guidance where necessary.
· Ensure that nursing documentation is completed accurately and in line with regulatory requirements.
· Actively participate in MDT meetings, contributing clinical expertise and ensuring nursing perspectives are represented.
2. Patient Care
· Conducting initial assessments of new service users
· Ongoing care plan reviews and risk assessments, revise care plans as necessary, collaborate with other healthcare professionals.
· Respond to emergencies and complex patient needs, offering expert advice and interventions.

· Must also have ICU experience
· Occasionally providing direct care to service users to maintain hands-on clinical skills (e.g. tracheostomy change / sign off carer competency at same time)
3. Compliance & Quality Assurance
· Ensure compliance with all legal, regulatory, and ethical standards, including Care Quality Commission (CQC) requirements.
· Lead clinical audits, risk assessments, and quality improvement initiatives to enhance care quality and safety.
· Keep up to date with developments in nursing practice, health care, and legislation to ensure ongoing compliance with best practices.
· Ensure timely completion of medication audits, identifying any discrepancies and implementing corrective actions.
1. Training & Development
· Identify training and development needs of nursing and care staff and implement training programs to address those needs.
· Ensure that all staff are trained and competent in the use of equipment, techniques, and care interventions relevant to their role.
2. Operational support
· Work closely with the Homecare Manager to ensure the effective and efficient delivery of care services, managing resources, and ensuring staffing levels are appropriate.
· Contributing to the operational aspects of the division, including business development and liaison with commissioners, attending MDT meetings
· Act as a liaison between external healthcare providers, including hospitals, GPs, and specialists, to ensure continuity of care.
3. Communication & Relationship Management

· Foster a positive, supportive working environment, promoting teamwork and collaboration within the care team.
Essential Qualification & Experience
· Registered Nurse with a valid NMC registration.
· Significant experience in homecare, community nursing, or clinical settings, particularly with complex care needs.
· Excellent clinical assessment skills, with the ability to develop comprehensive care plans.
· Strong understanding of safeguarding, risk management, and infection control practices.
· Up-to-date with the latest healthcare regulations, clinical practices, and care guidelines.
· Excellent communication and interpersonal skills, with experience in MDT collaboration.
