Care Coordinator
Main Purpose(s) of job
e.g. to support division, to bring on new business, to maintain client relationships etc.
Key Tasks
e.g. making cold calls, data entry, recruiting staff etc.
- Develop and manage client relationships within the care team
- Complete monthly reports and care package schedules within deadlines
- Allocate shifts to the care team and promptly fill any schedule gaps with suitable staff
- Conduct quality monitoring of service users
- Participate in the on-call / standby rota
- You must be approachable with excellent customer service skills and experience; you will be a clear communicator with excellent telephone manners and highly organised and self-motivated.
Experience
Must have UK care- coordinating experience
UK healthcare experience is beneficial
2+ years experience as a Care Coordinator

KPI’s
- ON CALL / STANDBY Shifts (Out of Hours):
- Be available to rotational cover for ON CALL / STAND BY SHIFTS (Out of Hours) services when required, as part of the operational requirement of the role.
- KPI: Key Performance Indicators (KPI’s)
- The nature of KPI’s for this role is volume-based, reflecting the immediate operational requirement.
- As an introductory KPI framework, the following minimum expectations apply:
- first 4 weeks, co ordinating 5 packages & resourcing 5 candidates per month
- after 8 weeks, increase to 10 packages & resourcing 8 candidates per month
- after 12 weeks, minimum threshold coordinating 15 packages & resource 10 candidates per month
- KPI’s will be reviewed and may be amended by management to reflect the changing scope of evolving responsibilities and the operational needs of the business.
