Responsibilities:
- Request, obtain, and distribute medical records as needed.
- Perform intake and follow-up with foster parents and appropriate agencies to ensure youth receive necessary services.
- Assist with scheduling medical appointments for assigned youth.
- Answer phones and provide administrative support including faxing, scanning, copying, emailing, and filing records.
- Maintain accurate and up-to-date case files for assigned youth.
- Communicate relevant information to foster parents, medical case managers, medical offices, and DCFS case workers.
- Participate in internal and external audits of case files.
- Complete all required trainings in a timely manner.
- Perform additional duties as assigned.
Qualifications:
- Associate degree or a minimum of two (2) years of experience in a healthcare or social services setting required.
- Experience providing services to families with young children preferred.
- Ability to work independently with minimal supervision.
- Strong organizational skills with the ability to prioritize and multitask effectively.
- Excellent verbal and written communication skills.
- Strong attention to detail and accuracy.
- Proficient in Microsoft Office applications and electronic systems, including Word, Outlook, Excel, EHR platforms, and databases.
