Responsibilities:

  • Request, obtain, and distribute medical records as needed.
  • Perform intake and follow-up with foster parents and appropriate agencies to ensure youth receive necessary services.
  • Assist with scheduling medical appointments for assigned youth.
  • Answer phones and provide administrative support including faxing, scanning, copying, emailing, and filing records.
  • Maintain accurate and up-to-date case files for assigned youth.
  • Communicate relevant information to foster parents, medical case managers, medical offices, and DCFS case workers.
  • Participate in internal and external audits of case files.
  • Complete all required trainings in a timely manner.
  • Perform additional duties as assigned.

Qualifications:

  • Associate degree or a minimum of two (2) years of experience in a healthcare or social services setting required.
  • Experience providing services to families with young children preferred.
  • Ability to work independently with minimal supervision.
  • Strong organizational skills with the ability to prioritize and multitask effectively.
  • Excellent verbal and written communication skills.
  • Strong attention to detail and accuracy.
  • Proficient in Microsoft Office applications and electronic systems, including Word, Outlook, Excel, EHR platforms, and databases.
