NURSE PRACTIONER (NP) + SIGN-ON BONUS!


LOCATION: Missoula, MT

JOB TYPE: Full-Time, Permanent


As a Nurse Practitioner, you will take the lead in patient care at a modern primary care clinic serving older adults. This role focuses on delivering an exceptional patient experience through holistic, high-touch, coordinated care. You will manage your own patient panel, supported by a collaborative care team that includes Medical Assistants or LPNs, a Front Office Coordinator, a Clinic Manager, and a Medical Director.


Key Responsibilities:

Build a strong culture & foster community

  • Welcome all patients with empathy and compassion, ensuring they feel supported throughout their visit.
  • Listen carefully to each patient’s needs to provide an exceptional care experience.
  • Adapt to the needs of patients and the care team as required.
  • Delegate effectively to support staff for procedures, patient communication, and care coordination.
  • Contribute to a positive, engaged, and patient-centered clinic culture.


Deliver exceptional clinical care

  • Provide a full scope of primary care services, including wellness, acute, and chronic disease management.
  • Conduct comprehensive health assessments, make diagnoses, and prescribe medications per state regulations.
  • Perform Annual Wellness Visits and Health Risk Assessments with a focus on accurate and complete documentation.
  • Conduct patient visits in various settings, including clinic, telehealth, or home visits as needed.
  • Coordinate care across the patient’s healthcare team to ensure continuity and quality of care.
  • Order and interpret diagnostic tests, perform screenings and in-office procedures.
  • Educate patients on prevention, lifestyle management, and disease care.
  • Make appropriate referrals to specialists and follow up on outcomes.
  • Manage post-hospitalization and post-discharge follow-up care.
  • Provide telephonic triage and after-hours coverage on a rotating basis.
  • Complete prior authorizations and peer-to-peer reviews when necessary.


Ensure clinical quality and safety

  • Accurately capture health conditions through appropriate coding and documentation.
  • Meet or exceed key quality and clinical performance measures.


Promote continuous improvement and professional growth

  • Maintain complete and timely electronic medical records.
  • Participate in care team meetings, quality reviews, and chart audits.
  • Support onboarding and training for new providers or students.
  • Collaborate on operational initiatives, patient outreach, and educational events.
  • Participate in staff meetings and professional development opportunities.


Deliver a premier patient and team experience

  • Prioritize patient satisfaction and quality outcomes in every interaction.
  • Communicate effectively with all team members to ensure seamless patient flow and coordinated care.


Qualifications:

  • Active Nurse Practitioner license (FNP or AGNP) in good standing (required).
  • CPR/BLS Certification (required)
  • Prescriptive Authority and DEA License (required)
  • 3–5+ years of independent primary care experience (required)
  • Valid Driver’s License (required)
  • Proficiency with Microsoft Office Suite (required)
  • Adult Gerontology specialty certification (preferred)
  • Experience with value-based care and panel management (preferred)
  • Experience in launching or growing new practices (preferred)
  • Familiarity with Electronic Health Records (EHR) (preferred)
  • Experience providing patient or community education (preferred)
