Title: Primary Care Nurse Practitioner / Physician Assistant

Location: Danbury, CT

Schedule: Full-Time (5 days per week)

About the Role

We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide compassionate, proactive primary care to residents in skilled nursing and long-term care facilities. This is a full-time role that offers both professional fulfillment and the opportunity to make a lasting impact on the lives of seniors in the LaGrange community.

Responsibilities

• Deliver on-site primary care services in skilled nursing and long-term care facilities
• Conduct physical exams and comprehensive medical histories
• Diagnose, treat, and manage both chronic and acute conditions
• Order, review, and interpret diagnostic tests and lab results
• Prescribe and adjust medications as needed
• Create and update individualized treatment and care plans
• Collaborate with nurses, facility staff, and interdisciplinary teams
• Educate patients and families on treatment plans, prevention, and wellness strategies
• Maintain a supportive and respectful environment for residents

Qualifications

• Active and unrestricted NP, PA, or MD license
• Georgia state license in good standing (required)
• Minimum 6 months of experience in long-term care or skilled nursing
• Excellent clinical judgment, critical thinking, and communication skills
• Genuine passion for elder care and patient-centered healthcare
