This first-of-its-kind initiative closes critical gaps for patients living with end-stage renal disease (ESKD), improving access, reducing hospitalizations, and enhancing quality of life.

🏥 What You’ll Do
- Provide comprehensive primary care to patients within dialysis units
- Average 10–12 patient visits per day
- Coordinate care, order labs, and prescribe treatments for chronic conditions
- Conduct annual health evaluations and wellness visits
- Build strong partnerships with nephrology and integrated care teams
🎯 Qualifications
Required:
- 5+ years of independent practice in Primary Care, Internal Medicine, or Gerontology

Active VA Nurse Practitioner license (autonomous practice required)

DEA license and comfort managing chronic conditions (CHF, diabetes, hypertension, COPD)
- Master’s Degree or higher in Nursing
Preferred:
- Experience in dialysis or nephrology (not required)
- Experience launching or shaping new clinical programs
- Proven success prescribing and titrating complex medications
