Nurse Practitioner – Primary Care for Dialysis Patients

Locations: Chesapeake & Norfolk, VA

Schedule: Monday–Friday (No Weekends)

This first-of-its-kind initiative closes critical gaps for patients living with end-stage renal disease (ESKD), improving access, reducing hospitalizations, and enhancing quality of life.

🏥 What You’ll Do

  • Provide comprehensive primary care to patients within dialysis units
  • Average 10–12 patient visits per day
  • Coordinate care, order labs, and prescribe treatments for chronic conditions
  • Conduct annual health evaluations and wellness visits
  • Build strong partnerships with nephrology and integrated care teams

🎯 Qualifications

Required:

  • 5+ years of independent practice in Primary Care, Internal Medicine, or Gerontology
  • Active VA Nurse Practitioner license (autonomous practice required)
  • DEA license and comfort managing chronic conditions (CHF, diabetes, hypertension, COPD)
  • Master’s Degree or higher in Nursing

Preferred:

  • Experience in dialysis or nephrology (not required)
  • Experience launching or shaping new clinical programs
  • Proven success prescribing and titrating complex medications
