Job Title: Home Health Physical Therapist Assistant (PTA)

Job Type: Full-Time

Location: Albuquerque, NM

Reports To: Supervising Physical Therapist / Clinical Manager


Job Summary:

We are seeking a compassionate and skilled Physical Therapist Assistant (PTA) to join our Home Health team. The PTA will provide rehabilitative services to patients in their homes under the supervision and direction of a licensed Physical Therapist. The ideal candidate will possess strong interpersonal skills, a passion for patient care, and the ability to work independently in a home setting.


Key Responsibilities:

  • Deliver physical therapy treatments and interventions according to the plan of care developed by the supervising Physical Therapist.
  • Monitor and document patient progress and responses to treatment.
  • Assist patients with exercises, functional mobility, and use of assistive devices to improve physical function and independence.
  • Communicate regularly with the supervising PT regarding patient status, progress, and any changes in condition.
  • Educate patients and caregivers on treatment techniques, safety, and ways to manage daily activities.
  • Maintain accurate and timely documentation in accordance with agency policies and regulatory requirements.
  • Adhere to infection control and safety standards during in-home visits.
  • Participate in team meetings, in-service training, and case conferences as needed.


Qualifications:

  • Associate’s degree from an accredited Physical Therapist Assistant program.
  • Current licensure as a Physical Therapist Assistant in NM.
  • CPR certification.
  • Minimum of [1 year] of PTA experience preferred; home health experience is a plus.
  • Valid driver’s license and reliable transportation.
  • Strong communication and organizational skills.
  • Ability to work independently in a variety of home environments.


Working Conditions:

  • Requires travel to patients' homes.
  • Exposure to various home settings and environments.
  • May involve lifting, transferring, or assisting patients during treatments.
